Forms

All members must turn in Medical Forms Part A&B and a Waiver. Please fill out a set for each scout. Please also fill out a set for each adult who will be attending the fall or summer campout.

You must complete new forms every year. Last year's forms are no longer valid.

Forms are due by the beginning of October. Scouts and adults attending the Fall Campout must have completed forms on file.

Forms must be printed! We do not keep digital copies. You may complete these forms digitally, but then please print out physical copies.

Turn in completed forms to the Committee Chair.

The forms will be kept on file for the duration of the Scout year and will be available during any pack or den activity should an emergency arise. At the end of the scout year, the forms will be returned.

Pro-Tip

Fill the forms out digitally using a PDF editor. Then print and sign. That way, next year, assuming the forms don't change, all you have to do is modify the date.
